Introduction. Kidney doubling is a common abnormality of the upper urinary tract in childhood. The choice of surgical method of correction for obstructive uropathy in renal duplex system remains a controversial and debatable issue.
Objective. To analyze the outcomes of surgical treatment in patients with obstructive uropathy associated with complete renal duplication. Material and Methods. The study included 109 patients aged 0 to 18 years who underwent surgery for obstructive uropathy in duplicated kidneys between 2014 and 2023 at four Russian hospitals. All patients were divided into three groups based on the surgical technique used: ureterocystoneostomy (n=37), heminephrectomy (n=44), and ureteroureterostomy (n=28). Gender, age, and the distribution of obstructive uropathy were analyzed. The surgeries were evaluated based on the following parameters: time, presence of intraoperative complications, total number of postoperative complications, their severity distribution according to the Clavien-Dindo classification, and the number and extent of required reoperations.
Results. No intraoperative complications were identified. The highest number of postoperative complications was recorded in Group 1, while only Group 2 had grade IVa complications. Group 3 was characterized by the lowest number of complications and reoperations.
Conclusion. Laparoscopic ureteroureterostomy is an effective and safe method for treating obstructive uropathies of a duplicated kidney, allowing for the normalization of upper urinary tract urodynamics while demonstrating the least number of complications and repeated interventions.
